Integrated Services for Behavioral Health (ISBH) is a community-minded, forward-thinking behavioral health organization helping people along the road to health and well-being. We meet people in their homes and communities and help connect them to the resources. We serve Southeastern and Central Ohio with a comprehensive array of behavioral health and other services, working with local partners to promote healthy people and strong communities. Our services are intended to be collaborative and personalized for the individual.
OhioRISE is a Medicaid managed care program that provides specialized services for youth ages 0-20 with complex behavioral health and/or substance use needs.
The OhioRISE Care Coordinator is responsible for creating, planning, and organizing a team as defined by the family. This position facilitates child & family team meetings that support youth at home, in school, and in their communities with the goal of keeping families together and reducing hospitalizations. The team may include partnering with social service providers, community partners, and the families support systems. The care coordinators role includes leading the team in identifying needs, goals, and action steps to meet the families goals. The care coordinator ensures the youth and families have a voice and choice in all coordinated care and services provided.
The pay range for this position is $20.19 - $25.03 per hour based on experience, education, and/or licensure.
Essential Functions:
Uses a strengths based and solution focused mindset in all job functions and interactions internally and externally
Develop, lead, and facilitate team meetings for the young person and their identified team.
Provides care coordination within the familys home, community, or preferred location
Partners along with the young person, family, natural supports, and other service providers to identify care coordination needs/services to ensure the best outcomes for young people and families.
Maintains required documentation to access and maintain OhioRISE benefits
Meets quality and fidelity requirements as defined by ISBH OhioRISE standards
Continually acquires and maintains knowledge of community resources.
Attends mandatory in-person and virtual trainings and supervisions which will require travel
Completes all assigned trainings within identified timelines and maintains required
certifications
Follows ISBH policies and procedures in maintaining confidentiality, professionalism, and incident reporting
Utilizes databases and Electronic Health Record systems for accurate client documentation
Maintain compliance with HIPAA, mandated reporting requirements, and professional ethical guidelines
Travel within assigned service area
All other duties as assigned
Minimum Requirements:
Experience providing services and/or support to children and families connected
to behavioral health, child welfare, developmental disabilities, juvenile justice, or a
related public sector human services or behavioral healthcare field is required
Knowledge and experience in Hi-Fidelity Wraparound preferred (Certification provided
at time of employment) is preferred
Two years of experience in a coordinated supportive services or care coordination role
that required high efficiency, organization, and attention to detail is preferred
Experience working with people with autism spectrum disorders and developmental
disabilities is preferred
Experience in one or more of the following areas is required:
Have the qualifications and experience needed to work with children and families who are experiencing serious emotional disturbance (SED), trauma, co-occurring behavioral health disorders and who are engaged with one or more child serving systems (e.g. child welfare, intellectual and developmental disabilities, juvenile justice, education) is required
Able to effectively communicate through verbal/written expression required
Ability to use a personal vehicle or company vehicle to provide services in homes, schools, and community settings is required
Ability to maintain confidential information is required
Ability to adhere to all safety, rules, regulations and requirements is required
Ability to bill Medicaid without restrictions is required
A valid driver's license and the ability to operate a motor vehicle is required
An appropriate level of auto insurance coverage is required
Ability to manage deadlines within a fast-paced, high-volume environment is required
Ability to operate in an Internet-based, automated office environment is required
Ability to operate, receive and conduct on-camera video calls and/or meetings in an automated office environment is required
Ability to maintain a minimum of 100mbps for high-speed internet connection is
required
Physical Requirements:
Prolonged periods of sitting at a desk and working on a computer is required
Must be able to lift up to 15 pounds at times is required
